What to Bring
We ask that you bring your new patient forms (if you filled them out early), a valid state ID, and an up-to-date insurance card (if applicable). As far as your oral health history is concerned, if you have any previous X-ray imaging or documentation from past dentists, please ask them to fax or email your records to us. This helps us get a more comprehensive look into your health. If you have any questions about what you may need, please feel free to call us and we’d be more than happy to speak with you!
